Elder and Youth Manager

We are looking for a caring and enthusiastic individual who possesses excellent leadership skills to join our team. Working under the support and supervision provided by the UFN Health Director, must show expertise and commitment to working with front-line staff supporting elders and youth in the community. 

Responsibilities:

·        Work under the direction and supervision of the UFN Health Director

·        Creating and updating personalized care plans for patients.

·        Collaborating with patients' families, friends, and social supports in developing treatment plans.

·        Identifying and recruiting high-quality healthcare providers.

·        Interacting with healthcare providers to facilitate patients' treatments.

·        Ensuring patients’ treatment requirements are met.

·        Suggesting alternative treatment plans when patients' services requests do not meet medical necessity criteria.

·        Instructing and educating patients on procedures, healthcare provider instructions, and referrals.

·        Linking patients to social services programs and entitlements such as transportation assistance and translation services.

·        Conducting regular follow-ups with patients to evaluate progress, promote continuity of care, and ensure improved health outcomes.

·        Maintaining records of case management activities.

·        Keep all records in a locked filing cabinet.

·        Ensure Elder and Youth Plan is updated with goals, objectives, and activities in line with the UFN Health and Wellness Plan

·        Oversee planning and implementation of homecare staff programs and youth activities.

·        Work as a member of the UFN Wellness team

·        Other duties as assigned.

 

Required Knowledge Skills and Attributes:

·        Three years experience working individually with children, and elders in a First Nations community.

·        Strong leadership, organizational, communications, and teamwork skills

·        Budget development and experience

·        Supervising a team of clinical and non-clinical staff

·        Excellent computer skills, familiar with MS Office products, in particular Microsoft Word and Microsoft Excel

·        Maintain confidentiality and integrity showing respect to fellow employees and patients in the Ulkatcho Health Department and Anahim Lake Nursing Station and in the community.

 

Preferred Qualifications:

·        Training/experience in the field of Community Health, Homecare, Program Management in a First Nations community

·        Training/experience in developing and implementing activity programs, including recreational, life skills and community access and awareness.

·        Ability to effectively access community resources.

·        University Degree or College Certificate in a related field
