Wellness Facilitator
Position Summary
This Coordinator position encompasses Health & Wellness program deliverables including supporting Addictions, Mental Health, and Child & Youth needs and programs. The responsibilities include outreach of vulnerable persons from the Ulkatcho community who require assistance based upon a broad spectrum of needs.
Duties:
- Assisting with the enrolment of clients into detox and treatment centers across the province.
- Procuring & printing pamphlets and resources for clients.
- Connecting clients with resources to support their shelter, nutrition, vulnerabilities, .and employment.
- Data entry of Care pathway and Client log.
- Coordination for client appointments with services personal for needs of health, social services, judiciary, and cultural healing programs.
- Other duties not yet specified to support the Wellness team
Accountability
The coordinator will work under the supervision of the Ulkatcho First Nation, Wellness Director. The individual in the position will work as a team member and will assume responsibility and accountability to adhere to the policies, procedures, and strategic principles of the Ulkatcho Nation. Client confidentiality for this position will be strictly adhered to and any violation of this policy will be held accountable for.
Hours:
Full Time with a wage dependent on certification and experience
Qualifications:
- Degree in Social Services or other health or social service related field
- Good communication skills, both verbal and written
- Computer skills an asset
- Emotional control
- Ability to be flexible and adaptable
- Ability to travel
- Persons with knowledge and/or certification of Addictions preferred
